清远市外籍人士可以预约接种新冠病毒疫苗啦！
2021年5月20日起，清远市外籍人士按照“知情、自愿、自费、风险自担”原则，可以预约接种国产新冠病毒疫苗。
在广东省内年龄18周岁至59周岁，以及部分60周岁以上因特殊原因需接种且身体基础状况较好的外籍人士可自愿接种。
有广东医保参保凭证（社保卡或医保电子凭证）的外籍人士可免费接种。自费接种的，新冠病毒疫苗接种费按100元/剂次收取。
清远市目前使用的是国产新冠病毒灭活疫苗，全程需接种两剂。
清远市人民医院是清远市外籍人士唯一指定接种点，地址：清远市清城区新城银泉路B24号区。
（1）团体预约。5月20日起，如有需要，外籍人士所在企业、机构可向清远市人民医院进行集体预约。预约方式：在清远市人民医院网站(http://www.qyry.com/Item/9089.aspx)下载表格《外籍人员受种者信息导入模板》，填写相关信息后发电子邮件至639704807@qq.com预约，预约后等待接种通知。
（2）个人预约。5月20日起，申请人员进入“粤健通”微信服务平台，在外籍人士新冠病毒疫苗接种服务页面进行信息填报预约,或在清远市人民医院网站(http://www.qyry.com/Item/9089.aspx)下载表格《外籍人员受种者信息导入模板》，填写相关信息后发电子邮件至639704807@qq.com预约，预约后等待接种通知。

接种时外籍人士需携带中华人民共和国外国人永久居留身份证或护照及有效停居留证件。
已参加广东省社会保障医疗保险的外籍人士，现场需出示有效医保参保凭证。
在指定接种点接种前，请配合接种点工作人员完成规定程序，主动告知个人健康状况，配合医护人员询问，签署《知情同意书》和《免责承诺书》（扫描附后二维码可阅看），由专业人员判定是否符合接种条件。接种后留观30分钟，无不适症状后方可离开。在接种疫苗后14天内做好自我健康监测，出现身体不适情况应及时就医。

Foreigners Can Now Apply for COVID-19 Vaccination in Qingyuan
Foreigners in Qingyuan can apply for administering of COVID-19 vaccine produced by China, starting May 20, 2021, following the principle of "getting fully informed, voluntary participation, self-payment and self-responsibility".
Foreigners who are 18 to 59 years old, and some above 60 years old that need to be vaccinated for special reasons and have good physical conditions can volunteer to be vaccinated.
Foreigners who can present proof of Guangdong Provincial Medical Insurance Payments Records (Social Security Card or Electronic Medical Insurance Certificate) can be vaccinated for free. Other foreigners can get the COVID-19 vaccine at their own expense. The cost is RMB 100/ dose.
China’s domestically produced inactivated SARS-CoV-2 vaccines will be used, and two doses are required.
Qingyuan People’s Hospital is the only designated COVID vaccination site for foreigners in Qingyuan. Address: District B24, Yinquan Road, Qingcheng District, Qingyuan City.
(1) Group appointment. Starting from May 20, if necessary, foreign companies and institutions can make a collective appointment with Qingyuan People’s Hospital, download the form "Information Import Template for Foreign Recipients" on the website of Qingyuan People's Hospital (http://www.qyry.com/Item/9089.aspx), fill in the relevant information, send an e-mail to 639704807@qq.com to make an appointment, and wait for vaccination notice.
(2) Personal appointment. From May 20, applicants will enter the Yuejiantong WeChat service platform to fill in information and make appointments, or download the form "Information Import Template for Foreign Recipients" on the website of Qingyuan People's Hospital (http://www.qyry.com/Item/9089.aspx), fill in the relevant information, send an e-mail to 639704807@qq.com to make an appointment, and wait for vaccination notice.

You should present your permanent residence ID card for foreigners of the People's Republic of China, passports or valid residence permits at the vaccination site.
Foreigners who have participated in social security medical insurance in Guangdong Province should present a valid medical insurance participation certificate at the vaccination site.
Before vaccination at the site, please cooperate with the staff to complete the prescribed procedures, actively inform the medical staff of your health status, cooperate with the medical staff, sign both a form of informed consent and a statement of bearing personal responsibility for all risks associated with vaccination, and the professional will determine whether the vaccination conditions are met, and you should stay 30 minutes for observation on site after vaccination. Foreigners should also pay close attention to their health status within 14 days upon inoculation and go to the hospital promptly for medical advice in case of any adverse response.
